The gonfalon and the coat of arms were adopted on the 22nd June 1973. The coat of arms is argent four wavy fess azure. The gonfalon is blue with the coat of arms in the middle and the words Regione Basilicata in gold above.

Basilicata adopted on the 6 April 1999 a blue flag with the coat of arms of the region in the middle (argent four fesses wavy azure). The region adopted also an official regional day, the 20th May, which shall be the "Day of the Lucani of the world [Lucani=inhabitants of Lucania, which is the present region of Basilicata] " ("Giornata dei Lucani nel Mondo"). On that Day shall be flown the official flag of the region.

The arms and the gonfalone of "Regione Basilicata" have been adopted by law of 22 June 1973 (Bollettino Ufficiale, 25 July 1973). The text can be found in [zig86c]; the important parts follow (plus my translation):
"Art. 1 - Lo stemma della Regione Basilicata è costituito da una fascia di quattro onde di azzurro in campo argento.
Art. 2 - Il gonfalone è di colore azzurro mare, reca al centro lo stemma di cui all'articolo precedente e in alto l'iscrizione centrata in oro "Regione Basilicata". (...)"
[Art. 1 - The arms of the region Basilicata consists of a fess of four blue waves on a silver field. Art. 2 - The gonfalone is of sea-blue colour, and shows in the center the arms according to the preceding article and above the centered inscription in gold "Regione Basilicata". (...)]
The four waves represent the four major rivers of the region: Basenot, Agri, Bradano, Sinni. A sketch of the gonfalone was shown in [dlc93a].
A flag was first mentioned in [zig96b], but with the text "flag not known, copy of gonfalone". The flag sketch in this article indeed showed the flag with the inscription, as we had below as Variant. [vxi00c] finally mentions the official adoption of the flag and corrects the flag image (ommitting the inscription). The colour image shows a dark blue field and a bigger CoA (3/5 of the flag height as regulated in the law) with a thin yellow border.
